personally I would start with Michael Sylvester, the fit guru at bicycle gallery.
I also like Randy Hewitt, DC
and Stuart Stark, PT
All are bicyclists and have awareness of fit issues.
Ralph Yates, DO likes this work, but he wasn't a great match for me personally, other folks love him.

Also see the most recent Outside magazine that had an article about Andy Pruit and the like who are non-MD's doing high end (i.e. expensive) bicycle fit.
